At-Need Arrangements

For your safety, due to COVID-19, you can make funeral arrangements for your loved one from the safety and comfort of your home right here on our website. We are offering this as an option, along with being able to meet with us at our facility. If you would like to arrange the services of your loved one from a remote location, we have included this information to make it a seamless process for you and your family. In addition to the forms and information below, we will call you to schedule times and plan arrangements for your loved one’s funeral or memorial service.

The following is the needed information that we will use to prepare the obituary and the death certificate. Once you have completed the SCDHEC Worksheet, please fill out the remaining information provided on the list below. You may either email or send us a picture of this information.

  • Please complete this SCDHEC Worksheet. We will use this information for the death certificate so please be as thorough as possible
  • List of Survivors (If applicable)
  • Spouse
  • Children
  • Grandchildren
  • Siblings
  • Great-Grandchildren
  • Any others such as nieces, nephews, or significant others
  • List any church or fraternal connections
  • Special hobbies or remarks to be added in the obituary
  • Location the family will be if you are accepting visitors
  • Place for memorials to be sent in memory of the deceased
  • If services will be a traditional burial, please look at our selection of caskets and vaults
  • If choosing cremation, we will have a list of urns online soon but will email you a link or our urn selections for now.

You may email this to us as a document or you can write out the information and send us a picture once completed. Also, we can go over everything by phone if that works better for you.
